Queensland Athletics

Membership Application & Declaration

By submitting this application, I apply for membership of Queensland Athletics with the Club I have selected and declare that I am eligible to participate in athletics competition in accordance with the eligibility requirements set out in the By-Laws of Australian Athletics.

I agree to be bound by the Constitution, Rules, Regulations, Policies and By-Laws of Queensland Athletics, Australian Athletics and World Athletics, as amended from time to time, including all general and specific codes of conduct, integrity, disciplinary and anti-doping frameworks.

I acknowledge that participation in athletics activities involves inherent risks, and I voluntarily accept those risks. I agree to the waivers, releases, exclusions of liability and indemnities set out in the Australian Athletics Membership Application and Declaration – Terms and Conditions, which are available on the Australian Athletics website.

I consent to undergo drug testing under the auspices of Sport Integrity Australia, as required.

I consent to my name, results and image being used by Queensland Athletics and/or my affiliated Club for official publications, websites and communications, including images captured by official or approved Queensland Athletics or Club photographers.

Athletics North Qld

I hereby make application for membership of Athletics North Queensland Incorporated with the Club listed on this registration form and declare that I am eligible to participate in activities according to the eligibility laws set down by the By Laws of Athletics Australia.  I agree to abide by all the Rules and By Laws of Athletics North Queensland, Memorandums, Articles and By Laws of Athletics Australia and Constitution and Rules of the World Athletics as amended from time to time.  (Copies of this available on Athletics North Queensland website).  I consent to undergo drug testing under the auspices of ASADA. I consent to my name and/or image being used in publications, displayed on the website/social media sites of ANQ Affiliated Clubs & Athletics North Queensland, Queensland Athletics, Athletics Australia and World Athletics.

In consideration of your, or the person on whose behalf you are acting's, application for membership being accepted you acknowledge you have read, understood, and agree to the terms and conditions of membership as detailed in the Membership Application and Declaration – Terms and Conditions.

In order to proceed, you must have read and agreed to these terms and conditions of membership. By ticking the box below, you agree that if your application for membership is accepted, you will be bound by these terms and conditions.

I agree that I have read, understood, acknowledge and agree to the terms and conditions of membership, both on my behalf and on behalf of any person for whom I am lodging this application.

Or If the applicant is under 18 years of age, the following must be agreed to by the applicant's parent or guardian.

I am the parent or guardian of the applicant, and authorise and consent to the applicant undertaking the Athletics Activities. I agree to accept in my capacity as parent or guardian, the terms set out in this membership application.

Parent Volunteer Acknowledgment Statement

I hereby acknowledge that by registering on behalf of my child with North Mackay Athletics, agree to be present and assist in a volunteer capacity during club meets. I understand that volunteering is essential for the smooth operation of club events and commit to assisting with duties such as marshalling, raking, measuring, and child supervision as required.
